Assuming you cannot get the labs done without a doctor, maybe you could
tell your dermatologist that you were seriously considering
self-medicating and ask him if he would be willing to monitor your
blood levels. How do you think he would react?
And actually, that might indicate to him how serious you are about
solving your acne problem.
It's interesting that he is not willing to prescribe you Accutane but
he is willing to prescribe you a drug that has potential side-effects
-- rare liver problems, hyperpigmentation -- while simulaneously being
ineffective (in your case).
