I'm 39 year old and I suffer from acne since I was 18. Now the lesions to
the face are little, but once a month, a cist much large with pus break out
on face. On the back I still have lesions. My acnei s complicated by alse
rosacea.I 'm on antidepressants in small doses for problems of social
phobia (perhaps caused by acne). I'm in doubt if to try Roaccutane: I read
that it cause much dryness and redness of skin. My skin is very very
sensitive and thin and I hate to appear with the red ace (eritrophobia).
Take roaccutan or wait for some year in order to see if the acne gone alone?

Thank and sorry for my terrible English (I'm Italian)

I would guess that your acne is unlikely to disappear in the next year
if you've had it for 21 years already.

I think if the acne bothers you and is not well treated by other means,
Accutane is reasonable. Certainly, your case is fairly similar to mine
and I am currently on Accutane.

Accutane seems to be the way to go for you.

The stories you hear about side-effects are overblown and unnecessary
scare-mongering.

It was sometimes the case, years ago, that people would continue blindly
through a harsh accutane treatment as prescribed, but it is now recognised
that side-effects are very dose-related and can be controlled with simple
adjustments to the dose and extending the treatment period.

In my four courses and with my current low-dose control, the worst I had was
slightly dry lips, a tiny bit of cracking round the nose, slightly dry,
gritty eyes ( eyedrops help ) and dry nasal passages now and then. Nothing
major and a mere nuisance rather than cause for concern. My liver functions
and lipids also stayed in parameter.

Redness is not usually a problem. That is generally the result of topical
applications such as benzoyl peroxide or tretinoin, which are specifically
aimed at increasing skin-shedding to reduce keratin build up in the
follicle.

Topical Aloe Vera 99.9% gel is useful when on accutane; gentle soothing and
has a benefit to overall appearance.

You now have what is known as ' recalcitrant mature adult acne ' which, as
you have found out is resistant to standard acne treatments. This is why you
should seriously consider Accutane. You're an ideal candidate.

If you have a correctly controlled course, reaching the effective dose level
you could find you have clear skin beyond your expectations and the
difference that will make to your daily life will be well worth the cost of
achieving it.

Bite the bullet!

Regards
TJU

What TJU wrote below is right on. I ditto his remarks and urge you to
take assertive action in solving the problem. There are solutions,
don't sit back and wait.
