Skin surgery while on low-dose Accutane

Post subject: Skin surgery while on low-dose Accutane

So my many-month (mostly high dose) Accutane course finally appears to
be clearing my skin. I think I will soon have something worth
maintaining with a low-dose course (10mg/day).

There's a fly in the ointment. Over the last 8 weeks I've been
developing some sort of growth on the front of my cheek that my derm
says is either a mole or an angioma. I'd be happy to live with it if it
weren't so darn red and inflamed looking. It's not inflamed, it just
looks that way. Now that my skin is finally clearing up, I really don't
want something permanently installed on my face that looks like an
active pimple, even if it's not. The last time I saw my derm, he said
this would need to be cut out if I wanted it removed (unless he can
determine it's an angioma, which could just be zapped).

I'm trying to avoid any break in my Accutane course, because if the
acne returns I am not sure a low-dose course wiill do anything.

Thus, my question: Has anyone had any skin surgery while on low-dose
Accutane? I am hoping the skin on the face heals well enough when on a
dose as low as 10mg/day.

P.S. I did in fact have skin surgery while on Accutane back in 1998 (I
was on 120mg/day, or 1.8mg/kg). It was an adventure getting it to heal
(although it finally did), but that was on my back, and that was a
pretty high-dose.

Hmmmm... methinks I have an ingrown hair, not a new mole or an angioma
as my derm surmises. I say this even though its nowhere near the path
of my razor and has been growing for 8 or so weeks now.

It simply doesn't look anything like a mole or like any of the various
types of angiomas I've had zapped off in the past. It has a grey spot
in the middle and a flesh-to-red-colored bump around it. The bump
continues to get slightly bigger and slightly more red (or purplish).

I'm going to another derm on Friday for a second opinion. It will be
interesting to see if I guessed correctly. If so, that would be good
news: surgery would not be required to get rid of it (not that I would
need to get rid of it if it was a mole or angioma. I just don't like
the way it looks).

OK,

The second derm diagnosed it as a Eccrine Hidrocystoma. I don't think
that's any better than a mole, since it does need to be removed
surgically if I want to get rid of it.

Hi

Sounds like you have multiple causes here.
I've never come across an Eccrine Hidrocystoma, but it does remind me of a
persistent 'cyst' I had, now hopefully gone.

Every 4-6 months, despite having no acne because of my low-dose Accutane,
I'd get a 'cyst' on the right of my chin. It didn't respond to any topical
treatments, or a big increase in Accutane dose, seemed impossible to 'pop'
and would last as a reddened lump for two weeks, then fade and left a red
patch for another two weeks or so. I got so used to the little beggar that
as soon as I felt it start, I stopped shaving and grew a goatee to cover it
while it sorted itself out.

However, last time, I was so convinced that there must be a hidden
pus-bubble that I had a dig around, after a hot compress.

Seeing a tiny black speck deep in the raw tissue, I used a pair of splinter
tweezers and gripped it.
I was amazed to find I pulled out a thick whisker, much thicker than normal,
doubled over like a horse-shoe.
The end was split like a shaving brush.
I then bled like a waterfall!

It healed quickly, the redness faded, left no scar and it hasn't returned.
My guess is that this whisker just kept growing in circles and every now and
then my immune system was irritated enough by it to send in the Marines,
hence the red lump.

As always diagnosis is the Key!

You're right. A third derm diagnosed it as a new mole surrounded by a
proliferation of blood vessels (Telangiectasias?), resulting in
something like looks inflamed but is not. Anyway, he said it was not
really possible to know what it is for sure, but regardless the
treatment will be the same for all possibilities.

At that point, he whipped out some sort of device and zapped the
lesion. He knew I was on Accutane but said that was not an issue in
this case. There was no extra charge for the zapping: He charged me
only the consultation fee. I have a little white scab... it will be
interesting to see what appears when that scab heals.

It sounds to me like this went on for years. That would have driven me
nuts, thank goodness you could grow a goatee.

Well, that didn't work out. The little white scab from the zapping
finally fell off today (after 11 days) and the thing is still under
there, exactly as it was before, except redder.

I must give up on my plan for taking long-term Accutane so I can
eventually get this thing surgically removed (unfortunately, 6 months
to a year from now). That really sucks.
