A very effective antimicrobial ingredient that is available in many
shampoos is pyrithione zinc. It is in Zincon, DHS Zinc, some versions
of Head and Shoulders and many generic competitors. This ingredient
is slightly soluble in skin oils, so it hangs around for a day or
two. No need (or benefit) to use it twice a day. It is a skin
irritant if overused, and also suppresses hair growth with high
exposure.
Try coating his back with a layer of the shampoo a few minutes before
he showers (5 to 30 minutes), to allow time for the pyrithione zinc to
diffuse into his skin oil. Make sure he showers the shampoo off very
thoroughly, or his skin will feel tight and tender. Repeating on
alternate days is probably the best compromise between effectiveness
and irritation.
If this is going ot work for him, he will see improvement in a week or
two. It takes about a month to replace damaged skin, so full recovery
will take longer, even if this method works very well for him.
And get him some cotton tee shirts to try for a while. Each should be
worn one day only, before laundering.
