This contains some info about dosage recommendations:

I took accutane for a year and a half @ 40 + 20 mg a day. I am about
130lbs as well and had severe acne. I took my dosage all at once
allthough I noticed they do recommend splitting it up. It worked for
me, but my acne started coming back last year. I went back on it
again last year, and I cleared up with a much smaller dose and shorter
treatment period. The acne wasn't half as bad. I can't say you
shouldn't go over the recommended 60 mg for our body weight, cause I'm
not a doctor...but why don't you get a second medical opinion if you
aren't comfortable. Good luck!
