Here's an article on continuous low dose and ultra-low dose
maintenance courses of Accutane:

The doctor interviewed says it works for people with severe acne as
well as those adults whose acne has persisted since puberty.
I found the article interesting because I was on semi-low dose
(10mg/day) maintenance course of Accutane for nearly 3 years, but I
had never seen anything about this treatment in the press or on the
internet (except from postings in this newsgroup by people who were on
such a regimen). My skin was perfectly clear during the entire 3
years. I went off it in late 2000 when I moved to the west coast and
my new dermatologist convinced me that I could probably stop it with
no recurrence of acne.
Now that I've been off the low dose course for over 3 years and have
begun to experience problems, I am considered doing it again and then
tapering it down to an ultra-low dose course mentioned in the article.
